We issue copies of medical records:
Monday, Wednesday, Thursday, Friday from 8: 00-14: 00
Tuesday from 8: 00-16: 00
Photocopies of the medical history are made after the registration working hours, therefore it should be reported earlier in the registration of a given clinic.
The copy of the documentation shall be made available immediately, no later than within 2 days of submitting the application.
